Rosacea: causes, symptoms and treatment of rosacea on the face

Сouperose is a pink drop, the French surgeon Guy de Scholiac gave this name to skin pathology, describing the disease for the first time in the XIV century. Thanks to the English dermatologist Thomas Bateman’s light hand, at the beginning of the 18th century, the disease was given a new name - rosacea (acne rosacea, episodic erythema, pink acne), which has survived to the present day.

What it is? Rosacea is a disease characterized by erythematous and papulopustular lesions of the skin of the face. It belongs to the class of chronic dermatoses (see photo).

The genesis is due to angioneurosis, characterized by vascular disorders in the area of afferent innervation of mixed cranial nerves (trigeminal nerve), caused by the most diverse causal factors:

  • congenital vascular pathologies (angiopathies);
  • disruption of autonomic support and hormonal failure ;
  • disorders of psycho-emotional state;
  • focal secondary (focal) infections;
  • due to violations of the gastrointestinal system.

Any of these factors may cause disturbances in the regulatory function of blood circulation in areas of the peripheral vascular bed - small arteries, arterioles, capillary and venous vessels. This causes a disturbance in the cellular nutrition of the dermis and epidermis, dystrophic processes in the collagen fibers and the oily-hair apparatus, with the manifestation of an organism's response in the form of inflammatory processes and the manifestation of skin rosacea.

Aggravating agents - alcoholic beverages and hot food, the influence of solar radiation, heat or cold can serve as a provoking factor.

The signs and symptoms of rosacea are very diverse. The classic development of the disease is caused by several successive stages.

1) The initial stage is characterized by the formation of periodic redness on the face in the area of the cheeks, nose, mid-forehead and decollete. Any stimulant can cause them. The development of the disease leads to frequent bouts of redness, manifesting itself as characteristic areas of persistent bright red foci, gradually acquiring a bluish tint.

This process is due to the violation of the integrity of the superficial saphenous veins, appearing on the skin surface translucency of red and purple cobwebs (spider veins). On the face appears swelling and burning, the skin begins to peel off.

2) At the second stage of the development of the disease, the process of papulopustular lesion of the face begins. Nodular lesions (papules), pustules (pustules), group localization of acne appear on the hyperemic surface. Their disappearance does not cause scarring of the tissues, it leaves behind, only an imperceptible trace.

There is swelling in the upper zone of the nose and forehead. The sun's rays cause the formation of black closed comedones. Signs of irritation in the form of burning and stinging subside at this stage. A pustular pustules rapidly "crumble" over the entire face, chest, sometimes spread to the back. Very strong itching symptoms accompany the localization of ulcers in the head.

3) The third stage is the most extensive, the disease progresses. The thickening begins on separate skin areas of the face and forehead, covers the surface of the nose and ear lobes. The skin is covered with bumps, becoming similar to the structure of an orange peel. Frequent complications in the form of rhinophyma are observed only in men, manifesting themselves with pineal nasal growth. It is enlarged, covered with a small vascular network, becomes bluish in color.

The development of the process leads to the formation of deep grooves on the surface of the nose, dividing it into separate uneven parts. The eyelids and chin are subjected to changes and skin thickening, the forehead becomes pillow-shaped, the earlobes take the form of cauliflower. Women's skin is not subject to such changes, this is due to the possible protective effect of estrogen.

Very often, when rosaceous pathology affects the eyes. Sometimes even before the skin manifestations. Characteristic is the development of blepharitis, manifested by peeling of the hyperemic skin on the eyelids. Appear crusts in the corners of the eyes, developing processes of acute conjunctivitis. Severe processes can cause blindness.

Special forms of "pink droplets" (rosacea)

The clinical characteristic of rosacea is due to the various forms of its manifestation, which plays an important role in the proper selection of its medical treatment, taking into account the factor of provocative influence.

  • The form of conglobatic pathology is characterized by the development in the subcutaneous fat layer of the enormous growths of conglobatic globular eels. The development of this process is due to treatment with iodine preparations and sedatives containing potassium bromide.
  • The steroid form is a consequence of the prolonged use of local hormonal preparations with the inclusion of fluorine. Steroid rosacea on the face is very difficult to treat.
  • Rosacea lightning. Manifested mainly in women on the background of neuropsychiatric disorders, during pregnancy and in menopause. Represents a complicated variation of conglobate rosacea. The development is sudden and rapidly progressing. Spherical acne subcutaneous growths strongly disfigure the face. Low effective treatment provokes the development of depression and neurosis in women.
  • Rosacea edematous (lymphodema). Rare pathology. Manifested a strong purple swelling of the entire face. Pressing on the edema does not leave behind the fossa because the edema is triggered not by the accumulation of fluid, but by the strong growth of subcutaneous adipose tissue. The general health of the patient is not disturbed, only the disfigured face shape is strongly pronounced.

Diagnostic examination

Diagnostic examination for rosacea To confidently diagnose a disease, it is usually sufficient to examine the patient by a dermatologist. But for the selection of effective treatment requires additional examination. In complicated processes, with the formation of pustules and purulent vesicles, are appointed:

  • bacteriological seeding study on the identification of cutaneous microflora;
  • analysis of dermal scraping, to determine the possible pathogen;
  • general examination of the body, to identify provocative foci that contribute to the development of pink acne.

At the initial stage of development, the disease responds well to therapy, but in neglected periods, this will take time and a special, individual approach in the selection of treatment.

Methods for treating rosacea on the face are as varied as the causes causing and aggravating the disease are diverse. The purpose of some drugs is mainly aimed at reducing the development of rosacea on the face, the use of other drugs should eliminate the background nervous and gastric pathologies.

  1. In treating this disease, polyketids (tetracyclines) class antibiotics are drugs of tetracycline hydrochloride, doxycycline, or minocycline.
  2. Effective drugs are Metronidazole and its analogues, which have antibacterial properties and high ability to restore the mucous layer of the gastrointestinal tract.
  3. As a topical treatment, hormonal creams, liquid suspensions, gels, various regenerating creams and ointments - Skinoren, Metrogil, Rozamet, Rozex are used. Eliminate inflammatory processes, reduce the process of pustular eruptions, contribute to the smoothing of the skin.
  4. In the case of a pronounced process of vascular radiography, Ascorutin vitamin preparations are prescribed to help strengthen the vascular walls.
  5. The manifestation of severe inflammation and pruritic symptoms with rozsezei, drugs "Suprostin", "Tavegil" or "Fenkorol", having anti-allergic effect can calm the skin. Homeopathic remedies in the form of motherwort, valerian or sage are used to normalize nervous disorders.

Physiotherapy treatments for face rosacea with laser, high-frequency light, cryotherapy and carbon dioxide (dry ice) are widely used. Such techniques effectively contribute to the elimination of small and large vascular neoplasms, control inflammatory reactions, selectively denature affected vessels, stimulate tissue regeneration processes.

Rosacea diet recommendations

But, without correction of the diet with rosacea, it is sometimes not possible to stop the progressing process. Food should be regular and fractional, that is, be taken often, but in small portions.

From the diet should remove all the provoking ingredients - spicy, salted, smoked and canned food. Abandon sweets and some fruits - pears, grapes and citrus.

Refrain from alcohol, strong hot tea and coffee. The diet should include non-fatty meat and fish with heat treatment (steamed) with the addition of vegetable or butter, boiled potatoes and eggs, greens, vegetables and fruits (except prohibited), yeastless stale bread or crackers, cereals and milky products.

In severe cases, the doctor may recommend a diet with fasting. It needs to be discussed individually.
